Sunshine Health Helps Long Term Care Member Find the Right Home

After a series of falls, Anna was placed in a nursing home to get the care she needed for several chronic conditions. Anna’s spirits declined in the nursing home. Her daughter lives out of state, and Anna wasn’t able to get the personalized attention she craved. With the help of a Sunshine Health care coordinator, Anna moved into a small assisted living facility where her quality of life dramatically improved. “It’s been fabulous,” her daughter said. “My mother is never alone … it’s just like being at home.” See the video.
