Sunshine Health Pathway to Shine 

At Sunshine Health, we understand that the needs of children who are or have been in the child welfare system are complex and will continue to change as they grow into young adulthood. Using a trauma informed approach, we collaborate with care coordinators to create a care plan unique to each member. We are your partner in health, now and in the future.

Quality Healthcare

You can count on our care, which is accredited from the National Committee for Quality Assurance.

$50 Over-The-Counter Benefit

Buy products like cough, cold and allergy medications, first aid supplies and vitamins. 

 

Healthy Rewards Program

Earn reward dollars for completing certain preventive health activities, such as well child visits for some age groups and weight loss coaching. 

Care Grants

Up to $150 per child per year for services and supplies for social or physical activities, such as gym membership, swim lessons, sports equipment, art supplies and application fees for post high school education.

Transition Assistance

Up to $500 in one-time assistance towards safe and stable housing for young adults transitioning out of foster care at age 18 or extended foster care at age 21. 

 

Transportation

Unlimited to medical appointments; 3 trips per month for nonmedical needs.

 

Telehealth

Easy access to a doctor when you want to see one.

How to Enroll

Sunshine Health Pathway to Shine is a specialty plan that is available to individuals who meet specified criteria based on eligibility for Child in Care Medicaid. Please contact your local Community-Based Care Lead Agency if you wish to check your eligibility.

Enroll in Sunshine Health

Call a Choice Counselor toll-free at 1-877-711-3662 (TTY 1-866-467-4970) or enroll online. To enroll, you must have the Florida Medicaid number or Social Security number and date of birth for each person you wish to enroll.
