Florida Nonprofits Serving People with Chronic Conditions Can Apply for $5,000 Grants

Sunshine Health is offering Community Connections Grants to Florida-based nonprofits that support people and families who are dealing with chronic conditions.

Nonprofits fulfilling needs in this area can apply for a $5,000 grant. We will award 20 grants across Florida’s nine Medicaid regions.

Special consideration may be given to organizations that serve populations that don’t have access to other resources, such as in rural areas.

To be eligible, organizations must have 501(c)(3) status in Florida. Applications will be accepted from February 10, 2025, 9 a.m. Eastern to February 28, 2025, 5 p.m. Eastern. Applicants must answer all of the questions on the form to be considered. While no protected health information (PHI) or personally identifiable information (PII) will be exchanged, winning applicants must provide a summary report of those served and outcomes and impacts after the funds have been spent.

Please see the Chronic Conditions page on our website. There is also a box on the application where you can list other conditions that may not be included. These will be reviewed once your application is submitted.

How long does the grant process take?

Awardees are typically notified within 30 days after the application process is closed.

How long is the duration of the grant?

Until the funds are depleted, but no longer than 12 months.

If my organization is chosen for a grant, how long does it take to get the funds?

Payment may take up to 8 weeks after you submit all necessary onboarding paperwork upon winning the grant.

Can the funds be spent on non-Sunshine Health members?

Yes. Supporting people with Chronic Conditions helps all Floridians.

If we’ve received other grants or funding from Sunshine Health, are we eligible to apply?

That depends on the level of funding received from Sunshine Health, when and the amount.  For this grant cycle, if you have a current contract/or sponsorship of $10,000 or more, you are not eligible. However, if you received a recent $2,000 “Get to Know Us” grant, or any of the smaller grants or sponsorship since January 1, 2024, you are eligible.   Reach out to your community relations representative, or email SM_FL_CommunityConnections@SunshineHealth.com if you are unsure.

Will applying for this grant mean I can’t apply for other Sunshine Health grants this year?

If you are the recipient of a $5,000 Sunshine Health grant in this grant cycle you will not be eligible for other grants of $5,000 or below in 2025. However, if you apply and do not receive the grant, you are eligible to apply for our other grants throughout the year.

How do I complete the application?

  • Open the application.
  • Respond to all questions in the fields provided. Incomplete, handwritten or scanned applications will not be considered.
  • Attach the completed application to an email and send it to SM_FL_CommunityConnections@SunshineHealth.com.
  • A response email will serve as your confirmation that your email was received.
