Sunshine Health has launched our new Provider Demographic Updates Form, which is designed to make it faster and easier for providers to update their practice information. This will help patients find out about your practice, help you better serve our members and help the health plan better serve our providers.

Streamlined Features

Providers will be able to use this new resource to:

  • Add or remove service locations
  • Update panel status (accepting new patients/closing panels)
  • Update and submit licensure (Medical/DEA license, Medicaid ID, etc.)
  • Update contact information
  • Submit name changes
  • Submit W-9s
  • Remove practitioners from your practice
  • Request updates to practitioner specialties
