Evolent Cardiology Solution Program Codes for Ambetter Health

Prior Authorization for medical necessity and appropriate length of stay (when applicable) for Ambetter Health members has been delegated to Evolent (formerly New Century Health) and will be required for the following procedures (and codes listed below) in both inpatient and outpatient settings:

This program applies to all Ambetter Health products for members ages 19 years and over.

NOTE: The coding sets identified above are subject to quarterly review and additions/deletions based on Evolent's current cardiology scope and coverage. To account for these changes, or any inadvertent omissions directly related to the procedures on the Evolent clinical scope of services, codes may be added or deleted throughout the term of the program.
