My Health Pays® Rewards

Get rewarded for focusing on your health!

Earn My Health Pays® rewards when you complete healthy activities like  annual screenings, tests and other ways to protect your health.

Use Your My Health Pays rewards to help pay for:

  • Utilities
  • Transportation
  • Telecommunications
  • Childcare services
  • Education
  • Rent

Or, you can use them to:

  • Shop at Walmart for everyday items**

**This card may not be used to buy alcohol, tobacco, or firearms products.

Healthy Activities Eligible For Rewards (As of July 1, 2023)

  • $20: Tobacco Cessation Health Coaching Sessions
    • Ages 10 and up. Must submit a consent form, verbally pledge to stop tobacco use and complete all four sessions within six months of the first session. $5 reward after each completed session
  • $20: Weight Loss Health Coaching Sessions
    • Ages 10 and up. Must submit a consent form, verbally pledge to lose weight within 30 days and complete six sessions within six months
  • $10: Substance Use Health Coaching
    • Ages 12 and up. Complete three coaching sessions with a Care Manager in three months. Enrollment in Case Management and signed consent form are required.
  • $20: Post Behavioral Health Admission Follow up Visit
    • Complete outpatient follow-up appointment with a behavioral health provider within seven days after discharge from an inpatient facility.
  • $20: Notification of Pregnancy Form (first trimester)
    • Complete and sign a Notification of Pregnancy form within first trimester.
  • $10: Notification of Pregnancy Form (second trimester)
    • Complete and sign a Notification of Pregnancy form within second trimester.
  • $50: Substance Use Treatment for Pregnant Members
    • Complete five Medication Assisted Treatment visits before delivery.
  • $25: Comprehensive Diabetes Care
    • Ages 18-75 years. Complete HbA1c test and dilated eye exam once in the calendar year
  • $20: Annual Well Child Visit, ages 3-21 years
    • One visit per calendar year with PCP
  • $10: Annual Well Child Visit, ages 0-30 months
    • One visit per calendar year with PCP
  • $50: Prenatal Visit
    • Complete three prenatal visits
  • $20: Postpartum Visit
    • Complete one postpartum follow up visit between 7-84 days after delivery
  • $20: Childhood Immunization Status (Combo 3)
    • Age 2 years. 4 doses diphtheria, tetanus, and pertussis (DTaP); 3 doses inactivated poliovirus (IPV); 1 dose measles, mumps, and rubella (MMR); 3 doses haemophilus influenza type B (HiB); 3 doses hepatitis B; 1 dose varicella-zoster virus (chicken pox or VZV); and 4 doses pneumococcal conjugate vaccinations (PCV)
  • $20: Immunizations for adolescents (Combo 2)
    • Ages 10-13 years. 1 dose meningococcal vaccine and tetanus, diphtheria, pertussis (Tdap), 2-3 doses human papillomavirus (HPV)
  • $20: Lead screening in children
    • Age 2 years. Complete annual blood test for lead poisoning screening
  • $20: Mammogram screening
    • Ages 50-74 years: Complete annual mammogram screening
  • $20: HRA completion
    • Completion of HRA for new members within 60 days of enrollment
  • $20: HPV vaccine series
    • Ages 13-26 years. Complete 2-3 doses human papillomavirus (HPV) vaccine
  • $20: TDAP for pregnant women
    • One dose tetanus, diphtheria, pertussis (Tdap) vaccine during pregnancy

What is a Health Coaching Session*?

The above Healthy Activities are eligible for rewards. These include participating in Health Coaching Sessions. The sessions are completed over the phone to fit your schedule. During these sessions, our Health Coaches can help answer questions about how to improve your health. They can also provide education and support to help our members set goals and make better lifestyle choices.

To get connected to a Health Coach, please call Member Services at 1-866-796-0530 (TTY: 1-800-955-8770), Monday through Friday from 8 a.m. to 8 p.m. Eastern.

Take healthy steps to earn My Health Pays rewards.

Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete.  If you are earning your first reward, your My Health Pays Visa® Prepaid Card will be mailed to you.

This My Health Pays Rewards Visa Prepaid card is issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. The Bancorp Bank; Member FDIC. Card cannot be used everywhere Visa debit cards are accepted.

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact Member Services.

Once you receive your My Health Pays rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation

To check your balance, login to your Secure Portal account.

Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.

Sunshine Health’s Healthy Behaviors program aims to help members improve their health and wellness. The program also helps members manage chronic conditions and build relationships with their providers. Substance Use Disorder (SUD) Program

The SUD program helps members, their families and caregivers access treatment, schedule doctor visits, get medications and manage side effects. It also helps them understand how the disease affects their emotional health.
