To help your child get healthy and stay healthy, we can provide rides to your child’s medical appointments at no cost to you.
ModivCare provides transportation for Child Welfare Specialty Plan members. There are no limits to the number of trips during the year between medical appointments, healthcare facilities or pharmacies.

Yes. You, a relative, caregiver or facility staff member can schedule rides.

Call Ride Assist (Where’s My Ride) at 1-877-659-8421. This line is staffed 24 hours each day, seven days a week.
- Transportation to and from all your child’s medical appointments. This could be on a bus, a van that can transport people with disabilities, a taxi or other kinds of vehicles.
- Three round trips per month for non-medical purposes such as social outings or family visits. Members must be between the ages of 5 and 21 to use this service.
- We cover the following services for members who have no transportation:
- Out-of-state travel
- Transfers between hospitals or facilities
- Escorts when medically necessary
Prior authorization is required for any trips over 100 miles
Member must not have another means of transportation to qualify.
Yes. Reimbursement is available per mile. To qualify for reimbursement, you must call ModivCare at 1-877-659-8420 (TTY 1-866-288-3133) Monday through Friday between 8 a.m. and 5 p.m. Eastern. All reimbursement requests can be made up to 30 days in advance but no later than the day of the appointment. Back dated mileage reimbursements are not allowed.
When you call in your child's reservation, you should provide the vendor with the name and mailing address of the person to whom the reimbursement is to be made payable. Once the trip is verified, you will be issued a Reference Number. This Reference Number becomes your Mileage Reimbursement Trip/Job number. You will only be reimbursed for trips on your Mileage Reimbursement Trip Log that includes this number.
Your child must not have another means of transportation to qualify. Gas mileage reimbursement is an option if your child has someone that can assist with transportation. Members, spouses or parents/guardians cannot get gas mileage reimbursement.

Each log must be filled out correctly and completely before you will be reimbursed. Fields include the trip date, medical provider name and phone number, physician/clinician signature, and total number of miles driven associated with that particular visit. Each date of service must have a physician or clinician signature in order for the reimbursement to be approved and must be confirmed with the physician’s office before payment will be made.
