New Member Transportation Vendor
We are excited to announce that Alivi will begin providing transportation services on January 1, 2025. Don’t worry – services will not stop at any time during this transition. Rides to and from medical appointments will continue as scheduled.
Beginning December 16, 2024, contact Alivi by phone Monday through Friday, 8 a.m. to 5 p.m. Eastern to book a trip for rides on January 1, 2025 or after. Call 24 hours a day, 7 days a week for real-time transportation support.

Standing orders with Modivcare will be carried over to Alivi. No action is needed on your part.

There will be no change in the transportation benefit. You will get the same member-focused care and support as you do today.
Members will still get unlimited rides to and from medical appointments. You must not have another means of transportation to qualify.
Alivi provides non-emergency medical transportation. They will take you to and from medical appointments.
- Covered members who are at least 18 years of age
- Covered member’s parent, legal guardian, or authorized representative
- Health Plan Case Manager or other Health Plan representative and other medical providers and/or facilities
- Covered members under 18 who are parents scheduling care for a dependent
- No minimum age requirement if covered member is seeking family planning or pregnancy-related services

- Transportation to and from all your medical appointments. This could be on the bus, a van that can transport people with disabilities, a taxi or other kinds of vehicles.
- We cover the following services for members who have no transportation:
- Out-of-state travel for medically covered trips (prior authorization required)
- Transfers between hospitals or facilities
- Escorts when medically necessary
Prior authorization is required for trips over 100 miles.*
You must not have another means of transportation to qualify.
*Discharges to Florida location for ground trips subject to exclusion. All trips must be coordinated with Alivi.
