Care Management

Care Managers are trained to understand the unique challenges that you face. They offer the highest quality of care. Upon enrollment into the Sunshine Health Power to Thrive Specialty Plan, you will get a Sunshine Health care manager. Your care manager will help you with the following:

  • Education on healthy nutrition, medication compliance, and more.
  • Guidance on healthy behaviors, resources, education, closing gaps in care.
  •  Support: Addressing and solving any barriers you may face when it comes to attending doctor appointments and/or health screenings.
  • Health education on your  condition, including medication adherence and physician visits.
  • Building and making updates to your Care Plan as needed.
